Objective: The purpose of this study is to review the appropriateness of caspofungin use at Scott & White Memorial Hospital in an estimated patient sample size of 50 to determine the types of patients and infection for which caspofungin is being utilized, as well as treatment outcomes. Methods: Adult patients who received caspofungin will be identified and their caspofungin and other systemic antifungal drug utilization data along with medical data regarding inpatient stay will be collected. Each patient's case will be evaluated to determine if the use of caspofungin followed our updated appropriate use criteria and the direct cost of caspofungin therapy calculated. The percentage of cases compliant with use criteria will be calculated. Results: Twenty-two patients have been analyzed to date. About two-thirds had multiple sites of infection. Eleven had positive fungal cultures; Candida albicans being the most common isolate 50% ; . A majority of patients were on fluconazole just prior to caspofungin therapy 59.1% ; . Nguyen St. Luke's Episcopal Hospital, Houston, Texas Background: In 2005 a medication utilization evaluation MUE ; of 2, 725 patients revealed opportunities for improved utilization of darbepoetin alfa. Implementation of a pharmacy-operated darbepoetin optimization service was proposed and approved by the Pharmacy, Nutrition and Therapeutics PNT ; Committee and the facility's Institutional Review Board. Objectives: To conduct a pilot study assessing the feasibility of hospital-wide implementation of a darbepoetin optimization service by staff clinical pharmacists and to determine clinical and financial outcomes of this service. Methods: PNT approved guidelines regarding the initiation, monitoring, and discontinuation of darbepoetin was piloted for one month. Data collected included: average time spent per patient, financial impact, effect on length of stay, hematological outcome, blood transfusion requirements, and number of patients on iron replacement. Results: A total of 141 patients were assessed. Average time spent per patient was 34 minutes. A cost savings of , 826 was obtained during the pilot with an extrapolated annual cost savings of 0, 467. Hematological outcomes and blood transfusion requirements were improved in patients staying greater than 14 days n 38 ; but were not significantly different than the MUE outcomes. Iron replacement was given in 34% of patients in the pilot versus 15% in the MUE. Conclusions: Hospital-wide implementation of a darbepoetin optimization service using a dedicated pharmacist service model approximately 1 FTE ; in collaboration with all clinical staff pharmacists would provide significant cost savings and maintain clinical outcomes. In all such combinations, keeping statin doses to the minimum required is prudent. The most relevant drug interactions for the average patient with diabetes are those with fibrate drugs and niacin, agents frequently employed to reduce the hypertriglyceridemia and low HDL cholesterol levels commonly encountered in patients with type 2 diabetes and often poorly addressed with statin therapy. Recent studies suggest the possibility that the drug interactions with fibrates may be mediated through effects on statin glucuronidation, another mechanism of drug elimination.7 Fenofibrate seems to be much less frequently associated with these adverse events than gemfibrozil.8 In the setting of statin therapy, niacin doses should be limited to no more than 1 g or, at most, 2 g day. The risk of myopathy is almost certainly less with statin-niacin combination therapy than with statin-gemfibrozil combination. In fact, there is now a combination tablet of extended-release niacin with lovastatin available in 500 20, 750 and 1, 000 20 tablets with a maximum approved dose of 2, 000 mg of niacin and 40 mg of lovastatin.9.
Generic versions of gemfibrozil are available and cost to per 60 tablets.
The use of gemfibrozil together with mevacor is noted in pharmacology texts to be associated with rhabdomyolysis.
In most subjects who have had an unsatisfactory lipid response to either drug alone, the possible benefit of combined therapy with hmg-coa reductase inhibitors and gemfibrozil does not outweigh the risks of severe myopathy, rhabdomyolysis and acute renal failure!
